Flu Shots to be Offered at High
School Sept 24th
Walgreens of West Chicago will be offering seasonal (not H1N1) flu shots at=
the
high school this Thursday, September 24th, for students during
lunches and for parents from 5-9 p.m. Students over the age of 14 are
eligible to receive this vaccine with parental consent. Please use the
links below to print out the consent form, which must be signed by par=
ents
and returned to the Health Office at the school anytime between now and end=
of
the day next Wednesday.

Parents are invited to receive the vaccination between= 5 and 9 p.m. in the Administrative Conference Room, located inside Entrance H. The nasal vaccine and preservative-free vaccine, for pregnant women, will also be available. Parents, if you are interested in receiving the immunization, please contact Walgreen’s pharmacy at 630.293.8951.
The seasonal flu is free for those with Medicaid Part B. Anyone on Medicaid AND over the age of 19 may receive it with a $3.00 co-payment. For those without insurance or whose vaccines are not cove= red by their insurance plan, the cost is $24.99. Cash, check, or credit ca= rd is acceptable as payment.
