New Student Enrollment (for a student new to the district)

DOCUMENTS REQUIRED FOR NEW STUDENT ENROLLMENT
The following documents are required to enroll a student NEW to the district:
PARENTS MUST OBTAIN 4, 5 AND 6 FROM THE PREVIOUS SCHOOL
- Original birth certificate
- Residency Verification: Latest Tax Bill, Mortgage contract, Lease agreement plus 2 forms of ID with Name and current address. If these documents are not available or if the student could be considered homeless, please click here for further information.
- Legal guardian’s Driver’s license and custody verification
- Unofficial transcript and withdrawal grades
- Copy of immunization records and current IL physical
- Illinois student transfer form (Illinois transfers only)
- Copy of IEP (if applicable for Special Education, also can be obtained from previous school if you don’t already have your own copy)
- Copy of 504 Plan (if applicable for Special Education, also can be obtained from previous school if you don’t already have your own copy)
- Bilingual needs
- Request for Records form (see below for packet, please print & complete)
- Student Demographic form (see below for packet, please print & complete)
- Home language survey (see below for packet, please print & complete)
- Parent & Student read Student Discipline Policy (see below for packet)
- Parent & Student read the Computer Internet Use Agreement (see below for packet)
Once the above documents are attained please email them to flopez@d94.org to begin the registration process.
